Required Forms

All campers staying overnight at a camp at the University of Montana Basketball Camps must print, fill out, and bring a Doctor's Health Certificate to camp, or mail to:

Online Registrants will be emailed a Doctor's Health Certificate.  Campers that do not send in or hand in a Doctor's Health Certificate may not be able to participate in camp activities.
